Custom Standalone Forms are independent, flexible forms used to capture general or log-based data without linking it to clients or staff. They’re ideal for reporting, tracking, and standalone data collection that supports broader workflows.
Examples: Incident Report Form, Equipment Request Form, Feedback Form, General Data Log
[Permissions] The staff member must have permissions to the Form Builder module [Staff > Staff Info > System Permissions > Form Builder] in order to design Standalone Forms.
Who Uses Custom Standalone Forms:
- Administrative staff and coordinators to collect general organizational data or feedback not linked to clients or staff profiles
- System administrators to configure and maintain forms to meet various operational needs
Purpose of Using Custom Standalone Forms:
- Collect and manage independent data that is not tied directly to client or staff records
- Support organizational, operational, or project-specific data collection
- Provide flexible data capture that can be reviewed, reported on, and exported as needed

Let’s use a feedback form as an example to see how a Standalone Custom Form is created in Form Builder. This form captures general feedback that isn’t linked to client or staff records, making it ideal for collecting organization-wide input.
While this example uses a feedback form, you can create other Standalone Forms tailored to your operational or administrative needs by adding different controls and configurations.
Standalone Form Category: To create a Standalone Form, navigate to Form Builder and select the Standalone Form category.

Choosing a form design: Select a form design by cloning and editing an existing form, or by creating a new one from scratch.

Designing a new custom standalone form:
Create your Standalone Form by choosing controls from the left side of the Form Builder. Each selection comes to life on the central canvas, while its properties and settings are easily refined in the panel on the right.
This feedback form is an example of a Standalone Form, which collects independent data that is not linked to client or staff records. It uses the following field types:
- Textbox: Used to enter short text such as the feedback topic, contact number of the feedback provider, or the feedback provider’s name
- Date Picker: Used to select dates, such as the feedback date
- Dropdown: Used to select the feedback type (e.g., Complaint, Suggestion, Praise)
- Memo: Used to capture detailed descriptions or comments related to the feedback
- Required Field (*): Fields marked with an (*) must be filled out before the form can be saved
