Form Builder makes it easy to create and manage digital forms tailored to your needs. It replaces traditional paper forms with clean, organized online forms that are simple to build, update, and store. You can design forms for both staff and patients, helping ensure information is captured accurately and consistently.
With flexible options to customize fields, layouts, and sections, each form can be tailored to your specific workflows. This saves time, reduces errors, and keeps your documentation clear, consistent, and well-organized.

Form Builder includes several form types, each designed for a specific purpose. You can use different forms to collect patient information, record staff details, manage reusable data lists, or gather general information and feedback.
Each form type offers its own features and level of flexibility, making it easier to capture the right information in the right place. Choosing the appropriate form type helps keep your data organized, accurate, and easy to access when needed.
Patient Form: This type is for forms that are directly linked to patient records. When opening a Patient Custom Form from the Paperwork section, you will be prompted to select a patient before the form appears. After selecting a patient, you can either update an existing record or create a new entry.
Examples: Consent Forms, Treatment Plan Forms, and other patient-specific documents.Progress Note Form: This is a special type of Patient Form that specifically collects progress note information. They are accessed directly from an Appointment, making it easy to record observations, updates, clinical notes, and billable details during or after a visit. Using these forms allows you to track a patient’s progress over time, support accurate billing, and ensure that important clinical and financial information is documented clearly and consistently.
Examples: SOAP Note, SNAP Note, DAP Note.Simple Form: This type is used to manage lists of items or values that can be referenced across other custom forms. Serving as a centralized data source, it allows you to populate fields such as dropdowns or list boxes consistently in multiple forms, ensuring accuracy and standardization throughout your documentation.
Examples: If your clinic needs a “Relationship” dropdown in several forms, you can manage the list in just one Simple Custom Form. Any changes you make here will automatically update all forms that use it, saving time and keeping your data consistent.Staff Form: This type is for forms that are directly linked to staff records. When opening a Staff Custom Form from the Staff section, you will be prompted to select a staff member before the form appears. After selecting a staff, you can either update an existing record or create a new entry.
Examples: Staff Onboarding, Performance Review.Stand-Alone Form: This type of form is not linked to any specific patient or staff record. It is used for general data collection or documentation. When opened, you can access the form directly without selecting a patient or staff member, allowing you to create new entries or review existing records.
Examples: Incident Reports, Equipment Requests, Feedback Forms, etc.

[Q] Can I embed a custom form into a stock form?
[A] Yes, you can embed a custom form into a stock form, and it will appear as a separate tab within that form.
Open Form Builder and select your custom form
Click the “Configure Where This Form Will Display” icon
Choose the stock form where you want to embed your custom form
[Note] You can embed only one custom form per stock form, but a single custom form can be embedded into multiple stock forms.
[Q] Is it possible to make sure the client is filling out a certain custom form during patient portal registration?
[A] Yes. You can make a form mandatory for clients to fill out during registration.
Go to Form Builder and select the desired form
Click Form Details > Patient Portal
Ensure the setting "How Can the form be accessed in the Patient Portal?" is set to During Portal Registration or Both
Enable the option “Make this form required for Patient portal registration submission.”
[Q] I created a new patient form, but I don’t see it in the Paperwork menu. What should I do?
[A] Open Form Builder, select the form you created, then go to Form Details > Distribute Forms. Once distributed, the form will appear in the Paperwork menu.
[Q] Can I update a list in one place and have it reflect across multiple forms?
[A] Yes. Lists managed with Simple Forms automatically propagate updates to all linked forms, saving time and ensuring consistency.
[Q] How do I ensure consistent documentation across forms?
[A] Use managed lists for common dropdown values and standardize field names and layouts in your forms. Patient and Staff Custom Forms help ensure clinical or operational notes are recorded in a structured and consistent manner.
[Q] Can I edit a form after it has been created?
[A] Yes. Forms created in the Form Builder can be updated or modified at any time. However, if your form has already been administered, you'll want to be aware of how those changes impact existing records.
