The Vital Tracking form allows you to record and review key clinical measurements, such as body metrics, blood pressure, oxygen saturation, pulse rates, menstrual history, lab values, counseling activities, and related lab orders, all in one place.
[Accordion] Getting Started
Use the Vital Tracking form to record and display all key patient clinical measurements in a single, convenient view.
- General Information: Records the date the vitals were taken (required), waist circumference (in inches), and body temperature (in °F)
- Body Measurements: Records the patient’s height (entered in feet and inches, total inches, or centimeters) and weight (in pounds). The system then automatically calculates related values such as BMI and BSA
- Blood Pressure: Records systolic and diastolic measurements taken in sitting, standing, and supine positions
- Respiratory & Oxygenation: Records the patient’s oxygen saturation percentage and respiratory rate (number of breaths per minute)
- Menstrual History: Enter the Last Menstrual Cycle date
- Pulse, Lab Results: Enter the required details based on your clinic’s documentation needs. If the blood pressure or pulse rate entered is outside of normal parameters, the field will turn red
- Counseling Offered: Select the option(s) that reflect the counseling provided

[A] Yes, BMI and BSA are automatically calculated once height and weight are entered.

[A] Yes, you can enter height in feet/inches, total inches, or centimeters.
[Q] Why are some fields disabled or grayed out?
[A] These fields may be auto-calculated or controlled by system rules. If something seems incorrect.
[Q] Can I record vitals for past dates?
[A] Yes, you can adjust the vitals date in the General Information section to document past measurements. You may also record vitals at every patient appointment if necessary.
