[Accordion] Overview
The Staff Assignment form is used to track and manage all staff currently associated with a patient's case. It maintains a detailed record of active assignments while also storing historical information for accuracy and audit purposes.
[Note] You can also access Staff Assignment from the patient's Quick Info > More tab or the right-hand side More icon options when in various forms throughout the system.
[Permissions] The staff must have Full access to the Patient Info module [Staff > Staff Info > System Permissions > Patient Info].
[Accordion] Getting Started
This form allows you to maintain a detailed record of all staff currently associated with a patient's care, while also preserving historical information. To keep the records accurate, this feature allows you to enter the date when a staff member is no longer a part of coordinating care for the patient.
- Click/Tap on [[img:add>> lbl:Add New Staff Assignment]] button to add a new Staff Assignment.
- Staff (required): Select the staff member who will be assigned to the patient.
- Assigned Type (required): Specify the type of assignment or role the staff member will hold.
- Program: Select the program associated with this Staff Assignment.
- Anticipated Hours/Week: Enter the number of hours per week the staff member is expected to devote to this Patient.
- Date Referred: Enter the date the patient was initially referred to this staff member.
- Date Assigned (required): Enter the date the staff member was assigned to this patient.
- Date Unassigned: Select the date that the assignment ends.
- Unassigned Reason: Choose the reason the staff member was unassigned.
- Notes: Add any additional information about the Staff Assignment.

[Q] Can multiple staff members be assigned to the same patient at the same time?
[A] Yes, multiple staff members can be assigned to the same patient in the same, or different, roles.
[Q] What does the “Notify All Assigned” button do?
[A] The [[img:outgoing_mail>> lbl:Notify All Assigned]] button sends a notification to every staff member who is currently assigned to the task.
[Q] How does this form help with reporting?
[A] All active and historical Staff Assignments are stored, allowing reports on staff involvement, patient coverage, and Program participation over time.
[Q] Can I limit a staff member to only access the patients they have Staff Assignments for?
[A] Yes, go to the staff member's Staff Info record > Additional Options tab > check Only Access Assigned Patients. Selecting this option blocks access to any patient for whom this user is not currently assigned in the Staff Assignments section. This will allow you to enforce per-user security and define specifically which patients they are allowed to access.
