[Accordion] Overview
The Audit Log Viewer allows authorized staff to review detailed histories of changes made within the system. It provides a transparent record of user actions, including what was changed, when, and by whom, ensuring accountability, data integrity, and compliance with organizational and regulatory standards.
Through the Audit Log Viewer, users can quickly track edits to patient records, form submissions, schedule changes, and more, making it easier to investigate discrepancies and ensure accurate, up-to-date documentation.
[Permissions] The staff member must have permissions to View Audit Logs [Staff > Staff Info > System Permissions > View Audit Logs]
[Tip] If you see different labels than what is shown in this article (e.g., Patient vs Client), your system may have Custom Labels applied. You can verify and update these by locating "Custom Labels" in the Global Search.
[Accordion] Getting Started
Begin by selecting the criteria for the Audit Logs you wish to view. Available filters include Log Type, Date Range, User Name, Patient, and Action Type.

Log Type (required): Select the type of log you want to view. Note: You may not see some of the options listed below if your organization is not licensed for the corresponding features.
Log In Record: This audit log records the login and logout activity of all users in the system, capturing the User ID, Login Time, and Logout Time
Patient Information Access: This audit log tracks all access to protected health information (PHI), recording details such as user ID, activity type, record section, patient name, and time of access
Security Log: This audit log tracks all user account activities and security-related events, such as login failures, account status changes, and audit log access or purging

Emergency Access Log: This audit log tracks all instances of staff logging in using Emergency Access, including details such as user, time of action, accessed forms and reports, security settings, and justification.
[Note] This feature is tied to your clinic's subscription to Meaningful Use.
Clinical Decision Support Log: This audit log tracks all staff activity related to changes in the Clinical Decision Support Rule Setup, including the staff member, time of action, CDS description, and rule action
Patient Portal Access Log: This audit log tracks all Patient Portal access by patients or their authorized representatives, including login details, user information, and device/browser data
Patient Portal Activity Log: This audit log tracks all actions taken by patients or their authorized representatives in the Patient Portal, including login details, accessed pages, form names, actions performed, and validation status
Options Form Audit Log: This audit log tracks all staff activity involving changes to the Options form, including the user, time of action, change details, and validation status [Forms > Utilities > Options Form]
Staff Portal Access Log: This audit log tracks all staff logins to the Staff Portal, including login time, IP address, browser details, platform, and validation status
Staff Portal Activity Log: This audit log tracks all activities performed by staff members in the Staff Portal, including login details, accessed pages, form names, actions performed, and validation status
HIE Integration: Sent Appointments: This audit log tracks all appointment records sent through the HIE Integration, including details of the staff, patient, appointment information, transmission status, and time of action
Action (required): Select the specific type of action history you want to display
User (required): Filter the log to show history related to a particular user
Start Date/End Date (required): Specify the date range within which you want to restrict your audit log search

Affected Patient (required): Select the name of the patient whose audit log records you want to view
The Audit Log Viewer will then display all records that match the search criteria you have entered

[[img:Do_Not_Disturb_On >> lbl: Purge Results to File]] Because audit logs can accumulate large amounts of data, you may occasionally need to remove older entries. Always check your agency’s data retention policies before purging, as some require logs to be retained for several years.
To purge data and save it as a text file, apply the desired filters and click/tap [[img:Do_Not_Disturb_On >> lbl: Purge Results to File]] button. After confirming, the data will be exported to a text file and removed from the database. Note that the purge action itself is recorded in the Security Log and cannot be deleted.
[[img:search >> lbl:Advanced Audit Log Viewer]] Auditing can also be performed at a more granular level. You can view field-level details, such as who made specific changes in the Scheduler, Patient Contacts, Custom Forms, and other modules. To access this feature, click the [[img:search >> lbl:Advanced Audit Log Viewer]] button.

[Q] I am not an Admin user. Can I still access the Audit Log Viewer?
[A] Yes. Any staff member with the View Audit Logs permission can access the Audit Log Viewer.

[Q] Why don’t I see any results when I search the audit logs?
[A] Ensure that the selected filters, such as date range, user, or action type, are correct and not too restrictive. Try expanding the date range or removing unnecessary filters. If you still see no results, confirm that audit logging is enabled for the actions you’re searching for.
[Q] Can I recover purged audit data?
[A] No. Once audit logs are purged from the database, they cannot be restored. Always ensure you have successfully saved the exported text after a purge action.
[Q] What are Advanced Audit Logs?
[A] The Advanced Audit Logs feature provides field-level tracking, allowing you to see exactly which fields were changed and by whom in modules such as Scheduler, Patient Contacts, and Custom Forms. Refer to the Related Articles link below.
