[Accordion] Overview

The Rating Scales form is designed to seamlessly capture and manage patients’ assessment results. 

It enables clinicians to record both overall test scores and detailed subscale values, providing a comprehensive picture of clinical findings. With its built-in calculator, subscale scores are automatically summed to generate the total score, reducing manual effort and ensuring accuracy, consistency, and efficiency in data entry.

[Permissions] The staff member must have Full permissions to the Paperwork module [Staff > Staff Info > System Permissions > Paperwork]

[Accordion] Getting Started

This form starts by displaying patient lookup, and once a patient is selected, their records are displayed.

Click/tap the [[img:add>> lbl:Add New Test]] button to create a new record. From there, select the appropriate rating scale, enter the test date, and record either the total score or individual subscale scores. 

When subscale scores are entered, the form automatically calculates the total score for you.

  1. Test Results for Patient:
    • Test (required): Select the required rating scale test from the list
    • Date: Set this to the date on which the test was administered
    • Score (required): The sum of subscale scores gets displayed here
  2. Subscale Scores:
    • Click/tap  on [[lbl:Calculate Score]] button to calculate and display the total score by summing all entered subscale values

Adding a new Test Structure

  • Click/Tap on [[img:add>> lbl:Add New Test Structure]] button to add/create a subscale to the Rating Scale
  • Test Name (required): Enter the name of the assessment you want to add to the system
  • Score Name: These columns let you decide the number of subscales required and label each subscale as needed
    1. Click/Tap on [[lbl:add]] to add a subscale and enter your custom label
    2. Add all the subscales you need and save the task
    3. After saving, the added subscales will be displayed
    4. Enter the numbers you want for each subscale and click/tap on [[lbl:Calculate Score]]
    5. The total sum of all the subscales will be displayed as the score

[Q] Can I edit a Rating Scale record after saving it?
[A] Yes, Rating Scale records can be edited unless they are locked or restricted by system permissions.

[Q] What happens if a subscale score is missing?
[A] Only entered subscale values are included in the calculation. Missing subscales will not be counted toward the total score.

[Q] Can I add new Rating Scales to the system?
[A] Yes, new Rating Scales can be added by clicking Add New Test and then Add New Test Structure when creating a new record, if your role permissions allow it.

[Q] Are Rating Scales tied to specific patients?
[A] Yes, each Rating Scale record is associated with a specific patient and stored as part of their clinical record.
